On the Chinese side of the China-Russia-North Korea border, in the Yanbian, Korean Autonomous Prefecture, Ku Nam toils his days away as a taxi driver. If Ku Nam goes to South Korea and successfully carries out a hit he will get paid $10,000 USD. Ku Nam accepts the deal and leaves for Korea on a rickety fishing boat with $500 dollars for his expenses. When Ku Nam arrives in South Korea he carefully scopes out his target for days, while also searching for his wife. When the time arrives for Ku Nam to take out his target, a string of unexpected events occurs leaving Ku Nam desperately looking for a way out. Meanwhile, the cops, Korean mafia, and the Chinese mafia all frantically search for Ku Nam.

Park Bok Ja is granted a special vacation after her death, which allows her to go see her daughter, Bang Jin Joo, a university professor living abroad. Unlike her expectation, Bok Ja finds Jin Joo living in her old house running a local diner and becomes frustrated with questions that she is unable to ask. After spending time alongside Jin Joo, Bok Ja realizes that her daughter is struggling with the unresolved feelings about her, and tries to find a way to show her sincere feelings that she had never shared before.

Komari Sano's parents ran a small Tokyo factory that manufactured aircraft parts. With her parents' influence, Komari Sano dreamed of becoming a pilot. During World War II, her older brother died as a kamikaze pilot and her parents died during the bombing of Tokyo. Komari Sano became an orphan and lived with her aunt. Her aunt ran a ryotei (high end traditional restaurant). After the war, all aerial activities were initially banned in Japan. After long negotiations with the Supreme Commander of the Allied Powers, Sizuo Matsuki, who is the security chief of Civil Aviation Bureau, finally launched Japan's civil aviation. Komari Sano applies for the position of a cabin attendant.

When a rocket attack on a hydroelectric power station in Sarang is completed, the Chinese side sends the National Security Bureau's "Thunderbolt Team" to escort Chinese experts to support the repair, but unexpectedly there is a hidden agenda and spy game for the attack on the power station.

During the 3rd season that was broadcasted last July 2017, when Kousaku Aizawa (Tomohisa Yamashita), Megumi Shiraishi (Yui Aragaki), Mihoko Hiyama (Erika Toda), Haruka Saejima (Manami Higa), and Kazuo Fujikawa (Yosuke Asari) have guided the young new addition of life-saving emergency interns, Soma Natori (Daiki Arioka), Shunpei Haitani (Ryo Narita), Akari Yokomine (Yuko Araki), and the flight nurse Futaba Yukimura (Fumika Baba) and have shown growth, they again desperately face lifesaving in a realistic scene of medical treatment and had been drawn into favorable reputation.

This special reveals another hidden story which was not told in the 3rd season.

The crash of Doctor Heli has left a huge scar. Dr Haitani, who whenever rides the Doctor Heli and talks in the radio, gets flashback memories of the crash, and needless to say, shakes and trembles when talking. Mio Oshima, who was on her way to her wedding, was engulfed in an accident and lost her bridegroom. The two of them were inflicted with a major wound in their hearts.

In order to overcome PTSD (Post-Traumatic Stress Disorder), Haitani began receiving counsel from the psychiatrist, Shiori Ninomiya (Rika Izumi).

However, the site where lifesaving is needed cannot wait. There are successive calls requesting for the Doctor Heli. And the collapsing of the subway tunnel happened. Haitani, who cannot go to the scene, had no choice but to make desperate efforts in the hospital.

Can Haitani make a new step and overcome his PTSD?!
