    As a child Takuma is diagnosed with a heart condition that requires care from a cardiologist. He soon becomes friends with his doctors young daughter Mayu while being treated at the hospital. One day Mayu overhears her father saying that Takumas condition is serious enough that he most likely wont live beyond the age of 20. Takuma isn't aware of this himself, however, so at 8-years-old he makes a promise to marry Mayu when they turn 20. Later in life, Takuma becomes aware that he wont be able to keep this promise and he begins distancing himself from Mayu in hopes of protecting her.

    Almost defying description, Miike's film opens with found documentary footage about sperm-production in young males and then turns to a graphically violent recreation of the execution in 1865 of the 28-year-old Okada Izo, a low-born samurai who killed in the service of the anti-shogun rebel Hanpeita Takechi. Fast-forward through 15 decades of Japanese history: suddenly Izo's spirit possesses the body of a street-sleeper in an alley of Tokyo's financial district.

    The reincarnated assassin goes on a killing rampage through time and space, from the Warring States period to the day after tomorrow, slaughtering everyone from a Buddhist elder to the prime minister ("Beat" Takeshi in one of dozens of star cameos), not forgetting his own mother. Nothing can stop him, because Izo is negation itself: the contradiction spewed up by the "perfect system" that is Japan. Nothing is immune to his attack, except perhaps the godlike emperor (Matsuda Ryuhei) and the maternal goddess of mercy (Momoi Kaori). Standing to one side, watching and commenting in song, is Tomokawa Kazuki, a radical folkie from the 1960s.

But Dakeichi also told Yozo a strange prediction about his life ... that he will become popular with the ladies and draw beautiful pictures. During Yozo's high school years he moved to Tokyo. In Tokyo he meets Horiki (Yusuke Iseya), a guy six years older than him. Horiki is known for being a player and one day drags Yozo to the Blue Flower Bar run by Madame Ritsuko (Michiyo Ookusu). At the bar there is poet Chuya Nakahara (Go Morita) who proclaims "war is brown like the color of shit". Yozo doesn't say anything as he takes in the environment. Yozo starts to drink more often and starts to live the fast life like Horiki. Like Dakeichi's prediction, Yozo also becomes popular with the ladies and always has a woman next to him. But, Yozo starts to become sick of this lifestyle. At a cafe, Yozo meets Hisako (Shinobu Terajima) a women who lets him drink without paying. Yozo senses that Hisako feels a heavy sense of loneliness like himself. One day Yozo and Hisako attempts to kill themselves out in the Sea of Kamakura. Hisako dies, but Yozo survives. His family then takes Yozo to a sanitarium ...
