During the 3rd season that was broadcasted last July 2017, when Kousaku Aizawa (Tomohisa Yamashita), Megumi Shiraishi (Yui Aragaki), Mihoko Hiyama (Erika Toda), Haruka Saejima (Manami Higa), and Kazuo Fujikawa (Yosuke Asari) have guided the young new addition of life-saving emergency interns, Soma Natori (Daiki Arioka), Shunpei Haitani (Ryo Narita), Akari Yokomine (Yuko Araki), and the flight nurse Futaba Yukimura (Fumika Baba) and have shown growth, they again desperately face lifesaving in a realistic scene of medical treatment and had been drawn into favorable reputation.

This special reveals another hidden story which was not told in the 3rd season.

"Can Haitani ride the Doctor Heli now?"

The crash of Doctor Heli has left a huge scar. Dr Haitani, who whenever rides the Doctor Heli and talks in the radio, gets flashback memories of the crash, and needless to say, shakes and trembles when talking. Mio Oshima, who was on her way to her wedding, was engulfed in an accident and lost her bridegroom. The two of them were inflicted with a major wound in their hearts.

In order to overcome PTSD (Post-Traumatic Stress Disorder), Haitani began receiving counsel from the psychiatrist, Shiori Ninomiya (Rika Izumi).

However, the site where lifesaving is needed cannot wait. There are successive calls requesting for the Doctor Heli. And the collapsing of the subway tunnel happened. Haitani, who cannot go to the scene, had no choice but to make desperate efforts in the hospital.

Can Haitani make a new step and overcome his PTSD?!

Sasaki Sachiko works as a literary magazine editor. She is well respected by her colleagues due to her perfectionist ways. On Sachiko'd wedding day, her boyfriend Shungo disappears. Sachiko is dumbfounded by her boyfriend's disappearance.

The next day, Sachiko goes to work, but she can't stop thinking about Shungo. She realizes she is more traumatized than she thought. Sachiko decides to go to a restaurant. When her meal comes out, she forgets all about Shungo. Her experience makes her become a connoisseur of gourmet meals.

Kousaku Aizawa, Megumi Shiraishi, Mihoko Hiyama, Kazuo Fuikawa and Haruka Saejima have worked and pursued careers down different paths.

Kousaku Aizawa left the hospital emergency room. He now works as a brain surgeon. Megumi Shiraishi works hard as a staff leader for flight doctors at a hospital emergency room. Mihoko Hiyama wanted to become a flight doctor in th past, but she left the hospital emergency room and now works as an ob-gyn. Kazuo Fuikawa works as a emergency physician specializing in orthopedics and works with Megumi Shiraishi. Haruka Saejima still works as a nurse for flight doctors.

They each live fiercely and face new situations.

Seventeen-year-old Mugi Awaya and Hanabi Yasuraoka appear to be the ideal couple. They are both pretty popular, and they seem to suit each other well. However, outsiders don't know of the secret they share. Both Mugi and Hanabi have hopeless crushes on someone else, and they are only dating each other to soothe their loneliness. Mugi is in love with Akane Minagawa, a young teacher who used to be his home tutor. Hanabi is also in love with a teacher, a young man who has been a family friend since she was little. In each other, they find a place where they can grieve for the ones they cannot have, and they share physical intimacy driven by loneliness. Will things stay like this for them forever?

Sakisaka Kento is a lawyer with an outstanding career. He seems cool and handsome, but he is a zealous person who will confront things that are unreasonable and wrong head-on. Mr. Ugly and Ms. Beast is a quirky yet oddly endearing, romantic comedy about an "ugly" guy Kenta and "beastly" girl Yukie. The ST team at the Tokyo Metropolitan Police Department consists of eccentric members. Akagi works as an analyst on the ST team. He won't appear in front of others, but his colleagues trust him as a leader. Yurine is tasked to manage the ST team. He experiences difficulties in dealing with the team. Meanwhile, the ST team come into conflict with other detectives at the police department over an indiscriminate gun firing case and a grotesque murder case. This forces Akagi to appear in front others. Akagi and Yurine work together to solve the case.

Machida Ritsuko and Nonoyama Shuichi (nickname Non-chan) are two thirtysomethings who have been living together for the past 8 years. One day, while Non-chan has gone fishing for the day, Ritsuko invited her good friend Niinuma Mina over to their apartment for some girls' talk. Mina then asked Ritsuko if she and Non-chan are planning to get married yet, to which Ritsuko replies, "He has yet to propose". At that exact moment, Non-chan was actually outside the door, having just returned from his fishing trip. Shocked to hear their conversation, Non-chan becomes moody, and thus begins to question their relationship. Although they have many things in common, but he feels they seem to be having many difference in opinions too...

Two prisoners managed to escape after the police truck carrying them crashed. One of them ,is a gangster. The other called "Otowa YonKo" - our protagonist - is a con artist whose cheats money from criminals or terrorists. Accidentally, "Otowa Yonko" then becomes the new teacher for the nearby high school, under the new name "Hachisuka Goro", and according to a deal of "Hammer Session" with the headmaster.
