Depicting the aftermath of " Kamen Rider Zero One REAL x TIME the Movie ", the four members of Mestuboujinrai; Horibi, Jin, Raiden and Naki, are on the path of coexisting with humankind. Together, Jin and Horobi continue to work so that malice cannot occur in the world, whilst Raiden works as an astronaut at the Space Business Center of Hiden Intelligence and Naki works as a technical advisor to the artificial intelligence special agency "AIMS" to crack down on AI crimes.

However, a man appears before Jin, announcing, "I have come to collect you" which causes Jin to become uneasy. Whilst the four are all together, Horobi states "This is a holy war", whilst raising his sword. Is Horobi turning his back on humankind after all? And who is the mysterious man with an interest in Jin?

"If God created this world in 6 days, then I will destroy it in 60 minutes, and create a paradise." Suddenly, a mysterious man named S / Kamen Rider Eden, accompanied by thousands of believers, appears. Meanwhile, several large-scale terrorist attacks take place worldwide simultaneously. Baba Hospital, the local clinic in a small port town, is closed for business. The clinic’s director Baba Kohei suddenly died and his only daughter Asuka, a nurse of one year, has been desperately searching around for a doctor to be his successor. Then Tsuruta Masayoshi, nicknamed Jumbo, suddenly shows up at Baba Hospital with an emergency patient in tow. He used to work with Asuka’s father at the hospital but left town because of some trouble 12 years ago. Although he saves the patient with admirable skill, he is not the fine young man he used to be but has transformed into a greedy, brutish and arrogant person.
